What to Expect from Our Home Care Services

Your return to your home begins at the hospital, where one of our nurses will meet you to find out about your care needs and the setting of your home. From there, the nurse will directly access important information to make your return home easier.

Our electric medical record integration helps facilitate continuous care coordination while you work towards your independence in the comfort of your own home with the company of your family members.

You’ll also have access to nurse navigators, who will provide you with ongoing support as you make progress and return to your feet.
